Student-Athlete Travel Departure Guidelines

Prior to any MSU team departure from the MSU campus for an away-from-home contest, this form must be completed in its entirety if a student-athlete or student athletics trainer will not be returning to the MSU campus with the original team travel party.
 
All required signatures must be obtained.  Once this form is completed, please forward to the MSU Compliance Office.
Incomplete forms will be returned.
